Meet the proud recipient of the first CCCTM of Distinction Award
The 2017 CCCTM of Distinction is Luke Emerson, BSN, RN, CCCTM, of Coeur d’Alene, ID.
Luke is a Clinical Navigator for Trauma Services at Kootenai Health in Coeur d’Alene, Idaho. He was nominated for this award by Dershi Bussey, BSN, RN.
Luke received his CCCTM credential in February 2016, and is the first recipient in his state.
He has been a guest speaker at North Idaho College, in Coeur d’Alene, ID, as an expert in care coordination and transition management (CCTM™). He also assisted in developing the care management portion of the associate degree nursing program at the college.
He believes that in addition to the medical treatment of an illness or injury, health care includes effectively managing the coordination of care and transitions.
Through his bi-annual presentation, “The Nurse as Manager of Care”, Luke educates the nursing staff at his facility on how to involve the multidisciplinary team to improve care coordination, and how to ensure successful transitions from within the hospital to outside of the hospital. He also advocates for the need for CCTM in his role as an Emergency Room Nurse Officer for the U.S. Army Reserves.
Luke continually supports certification by overseeing activities throughout the year that acknowledge certified nurses as well as those who receive a new certification.
